The frequency of peer specialist engagement allows for continuous assessment of changes in social determinants to a degree not typically attainable by more conventional means. MAP Care Solutions Certified Peer Recovery Support Specialists continually gauge a patient’s standing in these categories of social determinants through a combination of assessments and support related dialogue. These themes are continually revisited to allow the MCS team to understand a patient’s perception of social determinant related factors and progress against improving conditions thought to impact outcomes.

Today, the MAP Care Solutions Peer Recovery Support Specialist team is trained in talking through a patient’s struggles and educated on available options. The data captured, along with experiences of the PRSSs team from working with patients are combined to create additional educational opportunities.
