Peer-led Solutions for Substance Use Disorder


MAP Care Solutions is MAP's internal care management team that consists of primarily peer recovery support specialists and counselors. Since 2011, MAP Care Solutions has engaged in over 200,000 post-acute telehealth sessions and collaborative, post-acute care. Over the course of the last six years of gathering data and following persons in recovery from addiction and/or Substance Use Disorder longitudinally, MAP’s clinical teams have identified several important factors such as family history of addiction and having a co-occurring behavioral health disorder, which aid in determining the potential risk of a use event, relapse, or return to active addiction. These factors are incorporated into the predictive risk analysis and long-term support model.

MAP Care Solutions engages members and their consents, including family members, to facilitate deployment of MAP Care Solutions Services (also known as Recovery Support Services). These services include adaptive engagement programs which entails dynamically administering a combination telephonic and video-based telehealth, remote monitoring devices (such as breathalyzers, anxiety monitoring patches, medication adherence devices) and other technologies to maximize engagement rates with members and their consents / family.

Peer recovery support programs offered through MAP Care Solutions create better engagement, lower utilization of high cost services, and improve member experience. The goal is to ensure the member receives comprehensive and ongoing assessment, peer recovery support, subsequent care direction, and an actionable recovery plan to reduce the potential of relapse and promote the best quality of care and outcomes. The highlights of the peer support program are provided below:

  • Weekly, bi-weekly or monthly peer to peer recovery support sessions - ~30 minutes in length
  • Member maintains relationship with same peer over time
  • Peers uniquely trained to deliver services and collect outcomes data
  • Candidate selection criteria and referral model with Payer’s UM/CM/DM team
  • Peer recovery support engages family members and other consents
  • Periodic risk assessments for Substance Use Disorder and related co-occurring behavioral health disorders
  • Engagement channel and schedule for peer recovery support sessions based on assessment
  • Use of telehealth and remote monitoring technology to provide the peer recovery support services to the members
  • Facilitate care coordination of members to Primary Care Provider and Psychiatrists as directed by the assigned care managers of the member
